Indicator 13 Data Collection Tool - Demographic Information

Instructions:

1. Download application to local directory on your computer
2. "Unzip" the downloaded, compressed archive (Ind13.zip) file using a decompression utility like Winzip. A free trial version of Winzip is available. Users of Windows XP or Vista can use Microsoft's built-in zip archive support.
3. Once retrieved from the zip archive, double-click application to open. This application is compiled to run on an Intel compatible PC.
4. Once opened, enter demographic information.
5. Click "Save Information" upon completion.

Data will be stored in Excel spreadsheet called output.csv in local folder where application resides.
